The horizontal doesn't generally do much in terms of feeling (some would disagree)
The vertical often does, mostly with the ball (assuming it's a barbell) rubbing the clit. Some say it causes loss of sensation over time.
Then there's a clit piercing... i.e. through the clit itself rather than the skin fold over it (hood). That can be VERY sensitive, in both good and bad ways. I (male half) enjoy giving these piercings and include it as part of play (I have enough equipment lying around to pierce 100 people!) - victims welcome!
