I really cannot think of anything worse than having a needle pushed through the end of my dick.

ok speaking frankly here and with a little experience, if its a 1.2 or 1.6 or 1.8 then it wont do much damage to yr teeth 
If u go for the stretching and sizes over 2mm then yes it gets more awkward and dangerous for yr teeth lol

Nutter is a word we use on a day to day basis here, with each other, with the kids etc, it's a "south-east" slang word and is far less offensive than a lot of other words. It's jokey in it's use almost and I've certainly never come across any mention of it being an offensive insult.
